Below is a link for the New Hire Documents, which contains the New Hire Manuals, that you must review, and a consolidated sign-off form for those manuals.
You must print and sign a copy of the Mt. Prospect Park District Manual Sign-Off Form, indicating that you have received and reviewed each manual listed. Bring this completed form with you to your meeting with your hiring manager; (s)he will guide you through the rest of the hire-in paperwork during your meeting.
